One copy will be kept in the school's main office and the second will be kept in the Health Office.
This information is a critical communication tool to contact parents during the day. The information includes the student's name and contact information for parents and other relatives, friends or neighbors whom the school may contact if parents cannot be reached.
Please contact the school office if any of the emergency information (phone or email address) changes during the school year.
The emergency contact sheet authorizes the school to contact any of the people on the form in case of any emergency.
New York State Public Health Law, Section 2164 Chapter 443 requires that ALL students must be immunized or in the process of being immunized prior to entering school. Any child not in compliance with this law will be expelled from school. Please contact the school nurse on the immunizations needed.
Children should have a yearly physical as a preventative medical step to safeguard against any serious illness. New York State requires health examinations for all new students and for students entering grades Pre-K or K, 1, 3, 5 and 7. Physical examinations for the upcoming year are acceptable if administered not more than twelve months prior to the commencement of the school year in which the examination is required. A copy of the Health Examination Form MUST be submitted to the school nurse for placement in the student's health record. All parents are URGED to call the school nurse (914-944-0826 (ext 2)) if and when a child develops ANY communicable disease (pediculoses (head lice), chickenpox, measles, ringworm, strep throat, tuberculosis, hepatitis, scabies, etc.). Precautions must be made to protect all students. If in doubt, please call the school nurse (914-944-0826 (ext 2)).
Each time a child misses school, he/she must bring a written excuse, dated and signed by the parent, stating the reason for his/her absence.
Under provisions of the State Education Law and as outlined in the New York State Nursing Practice Act, a school nurse, principal or other personnel may not dispense internal medications to school children, without proper authorization.
When it is necessary for a student to take internal medication during school hours, the nurse may cooperate with the family physician and parents. In order to do this, we must have the following:
This medication must be brought to school by the parent and not sent with a child on the school bus. Medication permission slips may be obtained from the school nurse.
